Clearwater Pinnacle Peak in Scottsdale, AZ is seeking a Memory Support Director to lead memory care services, coordinate assessments, and work with hospital teams to ensure high-quality resident care.

You will hire, train and supervise staff, develop comprehensive care plans, and ensure compliance with federal, state and local standards while maintaining residents' physical and emotional well-being.

Competenze

• AA or Bachelor’s degree preferred in nursing, human services, healthcare, or related field.
• Minimum five to seven years in senior living, healthcare, or related industry.
• Nursing coursework completion or RN/LPN/LV certification is highly desirable.
• Minimum two to three years of supervisory experience with staff training and budgeting.

Mansioni

• Conduct and coordinate memory care resident assessments before move-in and develop comprehensive care plans.
• Monitor resident health and alert leadership and families of changes as needed.
• Coordinate nursing services and other resident services for continuity of care.
• Work with departments to ensure residents receive available services to enhance quality of life.
• Act as resource/care manager for memory care residents and families.
• Contact physicians and families to obtain orders for care and represent the community.
