Job summary

Complete Care Management LLC is hiring a Memory Care Coordinator for the day shift. The role focuses on observing and coordinating care with nursing staff, conducting rounds, and educating staff and residents' families.

Qualifications

• Prior experience managing clinical staff and coordinating with residents/families.
• Ability to work independently or as part of a group.
• Computer knowledge.
• Previous long term care experience is required.
• Memory Care experience required.

Responsibilities

• Observes and coordinates care needs in conjunction with nursing staff.
• Conduct rounds throughout the shift.
• Provide education, as needed, to staff, residents and resident family members.
• Ensure staff are performing all duties in accordance with local, state, and federal guidelines.
• Other duties as assigned by Assistant Director of Nursing, Director of Nursing or Administrator.
