Job summary

Qualifications

• Graduate of an approved school for practical or vocational nursing.
• Current license by the State Board of Nursing.
• 1–3 years of experience in long-term care, skilled nursing, memory care, or related healthcare.
• Experience with residents diagnosed with Alzheimer’s disease and related dementias preferred.
• Current CPR certification is required.
• Dementia Care Certification preferred or willingness to obtain.
• Must be able to read, write, speak, and understand English clearly.
• Verification of TB test per state law.

Responsibilities

• Coordinate memory care programming and resident engagement activities.
• Assess residents’ cognitive, physical, psychosocial, and behavioral needs.
• Develop and revise individualized care plans with the interdisciplinary team.
• Monitor progress and adjust plans as needed; participate in family meetings.
• Provide guidance to nursing staff on dementia care strategies.
• Administer medications per physician orders and document accordingly.
• Assist in discharge planning and on-call nursing rotations.
