Job summary

PSL Memory Care is seeking a dedicated Memory Care Coordinator to oversee the Connections Memory Care neighborhood. You will manage engagement and coordinate non-clinical services to support residents in a nurturing community.

The role focuses on creating personalized plans for successful aging, monitoring resident satisfaction, and motivating independence in daily activities within a warm, compassionate team environment.

Qualifications

• Experience in memory care or elder care settings.

Responsibilities

• Oversee the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services.
• Coordinate engagement aspects of life for Memory Care residents, including development of personalized plans for successful aging in community.
• Ensure resident satisfaction and achievement of realistic goals; motivate residents to function at their highest level of independence.

Highlights of the Memory Care Coordinator Position:
