Job summary

River Grove Health and Rehab is seeking a compassionate Memory Coordinator to lead engaging, person-centered activities for residents in our Memory Care Unit. The role requires a background in Activities Director, COTA, or social work, and experience with Alzheimer's or other forms of dementia.

Qualifications

• Experience running an activities department in long-term care.
• Background as Activities Director, COTA, or in social work.
• Excellent communication, creativity, and interpersonal skills.

Responsibilities

• Plan and coordinate a structured activity program for dementia residents.
• Facilitate group and one-on-one activities (music, art, reminiscence, life enrichment).
• Adapt activities to changing abilities; ensure all participants engage at their comfort level.
• Provide emotional and social support; build trusting relationships and encourage participation.
• Collaborate with nursing and social services to support care plans and communicate updates.
• Maintain documentation of participation and observations.
• Create a positive, home-like atmosphere in the Affinity Care Unit.
• Contribute to special events and family involvement activities.
