Guided by our core values of Passion, Respect, and Excellence, the Memory Care Activity Coordinator is responsible for developing and leading specialized programs that promote cognitive stimulation, emotional well‑being, and resident‑centered experiences.
Coordinate and oversee the development of unit objectives, policies, and procedures for the Memory Care program.
Prepare and present records and reports to the Executive Director regarding program budgets, progress, and goals.
Conduct admission and discharge assessments and develop individualized plans of care.
Develop and implement engaging daily programming designed to support residents’ cognitive, emotional, physical, and social well‑being.
Collaborate closely with staff, residents, and families to ensure open communication and personalized care experiences.
Pursue continuing education in dementia care and related fields to enhance program quality and best practices.
Lead, motivate, and support a team of compassionate caregivers to deliver exceptional resident experiences.
Maintain a therapeutic, structured, and enriching environment that promotes dignity and quality of life for residents with memory impairment.
An active CTRS certification in the state of PA is required.
Prior recreation or activities experience in a nursing home or Memory Care setting preferred.
Demonstrated knowledge and skills necessary to provide care appropriate to the age‑related needs of residents served.
Ability to communicate and interact tactfully with residents, family members, staff, visitors, government agencies, and the public.
Strong organizational, leadership, and interpersonal skills.
Passion for working with seniors and supporting individuals living with dementia and memory‑related conditions.
