Heritage House Rehabilitation & Healthcare in Indiana is hiring a Memory Care Activities Assistant on a part-time, days schedule. Qualifications

• Demonstrates CARE values to residents, families, customers and staff.
• High school diploma or GED; or 1–3 months related experience/training.
• State-approved CNA training or CNA program may be provided by the community.

Responsibilities

• Plan and lead memory care activities that engage residents.
• Collaborate with the care team to support daily routines and safety.
