Job summary

Elevate Care is seeking a Memory Care Life Enrichment Coordinator to plan, implement, and evaluate recreational and social programs for residents based on assessments and care plans. The role emphasizes collaboration with Memory Care leaders and other departments to ensure engaging, respectful activities that support residents' well-being.

Responsibilities include developing plans, documenting progress, and maintaining a warm, calm environment.

Qualifications

• Experience in Long Term Care Activities Department is preferred.
• Patience, tact, cheerful disposition & enthusiasm for working with residents.
• Ability to read, write, speak, and understand English.

Responsibilities

• Develop and plan activities in coordination with the team.
• Assist in planning, implementing & evaluating activity programs.
• Observe resident engagement and document progress.
• Maintain accurate activity notes and resident responses.
• Foster collaboration with other departments to ensure smooth activities.
• Prepare activity spaces and supply needs for programs.
• Support residents with crafts and daily activities as needed.
• Follow care plans to provide daily activities for residents.
