Job summary

River Grove Health and Rehab seeks a compassionate Memory Coordinator to lead dementia-focused activities in the Memory Care Unit. You will design engaging programs, coordinate group and individual activities, and tailor sessions to residents’ abilities while collaborating with the interdisciplinary team to maintain a warm, home-like environment.

Qualifications

• Experience leading activities in a long-term care setting.
• Background as Activities Director, Certified Occupational Therapy Assistant, or in social work.
• Strong communication, creativity, and interpersonal skills.
• Empathetic, patient, and able to adapt to memory loss.
• Ability to collaborate with an interdisciplinary care team.

Responsibilities

• Plan and coordinate a structured dementia activity program.
• Facilitate group and 1-on-1 activities (music, art, reminiscence, etc.).
• Adapt activities to residents' changing abilities.
• Provide emotional and social support and encourage participation.
• Collaborate with nursing and social services to support care plans.
• Maintain documentation of participation and observations.
• Create a positive, home-like atmosphere in the unit.
• Contribute to special events and family involvement.
