Job summary

Outcome HC is seeking a Memory Care Activity Assistant to support the Activity Director in planning meaningful activities for residents with dementia. You will help create a positive, engaging environment that promotes socialization, cognitive stimulation, and well-being while respecting each resident’s abilities.

Responsibilities include assisting with daily and group activities, transportation support, one-on-one activities, and documenting resident responses.

Qualifications

• Experience with memory care or healthcare preferred.
• Experience in long-term care is a plus.
• Ability to communicate with residents, families, and coworkers.
• Strong interpersonal and organizational skills.
• Patient, empathetic, and professional demeanor.

Responsibilities

• Plan, organize, and facilitate daily memory care activities.
• Promote cognitive stimulation, social interaction, and emotional wellbeing.
• Adapt activities to residents' abilities.
• Encourage participation while respecting choices.
• Assist with transportation to activities.
• Document participation and responses.
• Maintain a calm, safe, and structured environment.
• Communicate needs to the Activity Director and care team.
• Uphold HIPAA and confidentiality.
