Qualifications

• High School Diploma or GED required.

Responsibilities

• Understand and implement person-Centered thinking.
• Coordinate services and health benefits for members who meet criteria.
• Consult with members, families and legally responsible people to discuss health care needs.
• Monitor and evaluate the Person-Centered Service Plan on an ongoing basis.
• Maintain current and accurate documentation in the electronic record.
