Kroger Health Connect is seeking a Pharmacist for an on-site role in Cincinnati. Provide patient-centered services to optimize medication therapy from a centralized contact center. Qualifications

• Pharmacy degree required (BS-Pharmacy or PharmD).
• Licensed and in good standing with the State Board of Pharmacy in the resident state.
• Willingness to obtain additional non-resident licenses as assigned.
• ASHP-accredited residency or 2+ years of relevant pharmacy experience.
• Excellent written and oral communication skills for all stakeholders.
• Ability to prioritize and handle multiple projects and responsibilities.

Responsibilities

• Identify, prioritize, and resolve medication-related problems and determine appropriate follow-up care.
• Assist patients in identifying barriers to adherence and develop solutions.
• Engage patients and caregivers with effective communication and education techniques.
• Set therapeutic goals based on medications and disease states using guidelines.
• Provide professional recommendations to healthcare team members clearly and promptly.
• Demonstrate cultural competence and respect patient preferences.
• Teach and mentor pharmacy students on IPPEs and APPEs.
• Precept and mentor staff, interns, or residents.
• Ensure patient information and practice areas are compliant with regulations.
• Meet productivity and performance expectations per supervisor.

Job description

This role requires on-site attendance and does not offer a remote work option.

Provide patient-centered services that optimize medication therapy for improved clinical and economic outcomes and excellent patient care from a centralized contact center. Services will be provided telephonically and include, but are not limited to: Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, and other Enhanced Patient Education Services. Ensure accurate and efficient KHC service is provided to our patients, the pharmacies we service, and the providers we support. Responsibilities

• Identify, prioritize, and resolve medication-related problems, determine appropriate follow‑up care, and prevent new medication therapy problems
• Help patients identify barriers to adherence and develop solutions to overcome those barriers
• Use appropriate and professional communication skills and effective education techniques to engage patients and/or caregivers in decision making
• Formulate therapeutic goals for patients or caregivers based on medications and disease states using knowledge of primary literature and guidelines
• Make professional recommendations to other healthcare team members clearly, concisely, assertively, and in a timely manner
• Display cultural competence, respect for diversity, and respect for patient preferences and needs when providing patient care
• Teach, mentor and supervise pharmacy students on their Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs)
• Precept and mentor other staff, interns, or residents
• Ensure patient information and all areas of practice are handled in a compliant manner
• Meet productivity and performance expectations set forth by supervisor
• Demonstrate understanding and commitment to the purpose, vision, and mission of Kroger Health
• Apply effective workload and time management skills and document all services in a timely manner

Qualifications

Minimum

• Pharmacy Degree (BS-Pharmacy/PharmD) Required
• Must be licensed and remain in good standing with the State Board of Pharmacy in resident state

• ASHP-accredited residency or 2+ years relevant pharmacy experience
• Proven strength in written and oral communication skills to effectively relay information to patients, pharmacies, providers and to all levels of the organization
