Job summary

ADHEREHEALTH SOLUTIONS LLC is seeking a Medication Therapy Management Pharmacist to provide specialized patient reviews and drug therapy recommendations for insurer members.

The role includes telephonic comprehensive medication reviews, updating patient profiles, reviewing Drug Therapy Problems, and collaborating with a multidisciplinary team to optimize therapy. Requires Doctorate of Pharmacy or equivalent, active license, and bilingual proficiency in Korean and English.

Qualifications

• Doctorate of Pharmacy degree or equivalent.
• Active, unrestricted Pharmacy license.
• Must be fluent in Korean and English.

Responsibilities

• Performs telephonic comprehensive medication reviews.
• Updates patient profiles based on information gathered during the review.
• Reviews Drug Therapy Problems to optimize therapy and minimize adverse drug reactions.
• Works with multidisciplinary team to enhance and develop MedPro software.
• Completes at least two CMRs per hour.
• Assists Clinical team in meeting proficiency and quality goals.
• Utilizes client formulary information to guide medication recommendations.
• Keeps current on emerging trends to educate clinical team, physicians and other prescribers.

General Purpose

Medication Therapy Management Pharmacist provides specialized patient review and drug therapy recommendations for insurer’s eligible members.

Essential Duties & Responsibilities

• Performs telephonic comprehensive medication reviews
• Updates patient profiles based on information gathered during the review
• Reviews any Drug Therapy Problems to optimize patient’s drug therapy and minimize the potential for adverse drug reactions
• Works with multidisciplinary team to enhance and develop our MedPro software.
• Completes at least (2) two CMR’s per hour
• Assists Clinical team in meeting proficiency and quality goals
• Utilizes client formulary information and member specific benefits to guide appropriate medication recommendations
• Maintains current on emerging trends that will educate clinical team, physicians and other prescribers

Leadership Duties (If Applicable)

Establish positive partnerships across business groups both inside and outside of the organization

Education & Experience

• Doctorate of Pharmacy degree, or equivalent
• Active, unrestricted Pharmacy license
• Secure high-speed internet access (Ethernet connection preferred)
• Prior MTM experience preferred, but not necessary
• Must be fluent in Korean and English

Minimum Physical Requirement

• Regularly lift and/or move up to 10 pounds
• Bend, twist or stoop on a repetitive and continuous basis
• Stand and remain on feet for extended periods of time
• Operate various business instruments, including computers, printers/scanners/fax machines, requiring manual dexterity
• Sit and concentrate for long periods of time to compute, examine, and interpret data
• Read paperwork and computer monitors, requiring visual acuity
