Job summary

Kroger Health is seeking a pharmacist for an on-site role at the University of Cincinnati. You will provide patient-centered services that optimize medication therapy through a centralized contact center, including MTMs, adherence interventions, and education to patients and caregivers.

You will collaborate with pharmacists, providers, and the care team, maintain high professional standards, and mentor IPPEs and APPEs while ensuring compliant practice and timely documentation of services.

Qualifications

• Pharmacy degree required (BS-Pharmacy/PharmD).
• Licensed and in good standing with State Board of Pharmacy.
• ASHP-accredited residency or 2+ years relevant pharmacy experience.

Responsibilities

• Identify, prioritize, and resolve medication-related problems and follow up appropriately.
• Help patients overcome barriers to adherence and provide education.
• Communicate with patients, pharmacies, providers, and staff clearly and professionally.

Job description

This role requires on-site attendance at our University of Cincinnati location and does not offer a remote work option.

Provide patient-centered services that optimize medication therapy for improved clinical and economic outcomes and excellent patient care from a centralized contact center. Services will be provided telephonically and include, but are not limited to: Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, and other Enhanced Patient Education Services. Ensure accurate and efficient KHC service is provided to our patients, the pharmacies we service, and the providers we support. Responsibilities

• Identify, prioritize, and resolve medication-related problems, determine appropriate follow‑up care, and prevent new medication therapy problems
• Help patients identify barriers to adherence and develop solutions to overcome those barriers
• Use appropriate and professional communication skills and effective education techniques to engage patients and/or caregivers in decision making
• Formulate therapeutic goals for patients or caregivers based on medications and disease states using knowledge of primary literature and guidelines
• Make professional recommendations to other healthcare team members clearly, concisely, assertively, and in a timely manner
• Display cultural competence, respect for diversity, and respect for patient preferences and needs when providing patient care
• Teach, mentor and supervise pharmacy students on their Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs)
• Precept and mentor other staff, interns, or residents
• Ensure patient information and all areas of practice are handled in a compliant manner
• Meet productivity and performance expectations set forth by supervisor

Qualifications

Minimum

• Pharmacy Degree (BS-Pharmacy/PharmD) Required
• Must be licensed and remain in good standing with the State Board of Pharmacy in resident state
• Must be willing and able to obtain additional non-resident licensures as assigned
• ASHP-accredited residency or 2+ years relevant pharmacy experience
• Proven strength in written and oral communication skills to effectively relay information to patients, pharmacies, providers and to all levels of the organization
• Effectively able to prioritize and handle multiple projects and responsibilities
• Pharmacy Board Certification (Board of Pharmacy Specialties)
