Job summary

ICFAI in Statesville, NC is seeking a Medication Reconciliation Technician to review and verify a patient’s PTA medications within 24 hours of admission, leveraging pharmacy records and family input. The role focuses on accurate medication histories in the ED and Med Surg, with monthly performance reports and cross-team collaboration.

The position requires high school completion, a current Pharmacy Technician certification and NC Board of Pharmacy registration (or within 30 days).

Qualifications

• High school graduate or equivalent.
• Pharmacy Technician certification required (certification must be current and updated before the expiration date).
• Current Registration with NC Board of Pharmacy or registered within 30 days of hire.
• 5 years experience in hospital or retail pharmacy. A combination of both is preferred. Previous medication reconciliation experience in a hospital is desired.
• Ability to perform work accurately with attention to detail within specified time periods.
• Must be able to operate equipment used for the job. Ability to handle a high volume of work.
• Repetitive use of hands and fingers (e.g. preparing IV admixtures, use of computer).
• Lifting and carrying loads (up to 50 pounds) and ability to pass FIT test.

Responsibilities

• Review, clarify, document and verify PTA medications within 24 hours of admission.
• Notify Attending Physician when medication list is current and updated and alert them of changes, discrepancies, or additions/removals.
• Work primarily in the Emergency Department, with duties extending to Med Surg, Outpatient Surgery and Skilled Nursing Facility as needed.
• Maintain records of all medication reconciliation activities for performance monitoring and generate monthly reports on accuracy and timeliness.
