Job summary

HHS Hunterdon Healthcare System Inc. seeks a Medication Reconciliation Technician for the Emergency Department. You will collect admission medication histories, document them in the EMR, and support the pharmacy team with operations as needed.

Requires a high school diploma, NJ license, and 0–1 year of hospital or retail pharmacy experience. Preferred: ASHP/ACPE program, 2+ years retail, and CPhT certification.

Qualifications

• High school diploma or equivalent is required.
• ASHP/ACPE accredited program preferred; an associate degree or higher is a plus.
• 0–1 year hospital or retail pharmacy experience is expected; more is preferred.
• Current New Jersey Board of Pharmacy Technician License is required.
• Certified Pharmacy Technician (CPhT) is preferred.

Responsibilities

• Collects an accurate and complete home medication history by interviewing the patient and using secondary sources as needed.
• Enter medication history accurately into the EMR.
• Provide operational assistance in the ED or main pharmacy when time allows.
• Assist in training pharmacy technicians regarding medication histories.
