This crucial role involves overseeing medication administration and ensuring the safe and effective use of medications for residents. Key Responsibilities:

Pass medications on assigned shift.

Monitor medication administration to ensure accuracy, compliance, and resident safety.

Conduct routine medication and treatment storage reviews in accordance with policy and procedure.

Educate residents and caregivers on proper medication use, potential side effects, and adherence to treatment plans.

Collaborate with healthcare providers, including physicians, pharmacists, and other nurses, to optimize medication therapies.

Maintain accurate and detailed medication records and documentation.

Other duties as assigned by Health Care Director

Requirements

Requirements:

Active Licensed Practical Nurse (LPN) license in Pennsylvania.

Minimum of 2 years of clinical experience in a healthcare setting, with a focus on medication management preferred.

Strong understanding of pharmacology, medication administration, and drug interactions.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a multidisciplinary team.

Proficiency in electronic medication administration records (eMAR) systems.
