Job summary

Legend Senior Living in Lititz, PA is seeking a dedicated Medication Management Nurse to oversee medication administration and ensure safe use of medications for residents. The role requires strong organizational skills and meticulous attention to detail to support high-quality resident care.

The position involves collaborating with physicians, pharmacists, and other nurses, educating residents and caregivers, and maintaining precise medication records.

Qualifications

• Active LPN license in Pennsylvania.
• Minimum 2 years clinical healthcare experience (medication management preferred).
• Strong pharmacology knowledge and medication administration skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Proficiency with electronic medication administration records (eMAR).

Responsibilities

• Pass medications on assigned shift.
• Monitor medication administration for accuracy, compliance, and resident safety.
• Conduct routine medication and treatment storage reviews.
• Educate residents and caregivers on medication use, side effects, and adherence to plans.
• Collaborate with healthcare providers to optimize therapies.
• Maintain accurate medication records and documentation.
• Other duties as assigned by Health Care Director

Requirements

• Required: Active Licensed Practical Nurse (LPN) license in Pennsylvania.
• Required: Minimum of 2 years of clinical experience in a healthcare setting (focus on medication management preferred).
• Required: Strong understanding of pharmacology, medication administration, and drug interactions.
• Required: Excellent communication and interpersonal skills.
• Required: Ability to work independently and as part of a multidisciplinary team.
• Required: Proficiency in electronic medication administration records (eMAR) systems.

Preferred Qualifications

• Prefer: Experience in personal care, long-term care, hospice, or home health settings.
• Prefer: Knowledge of regulatory standards and guidelines related to medication management.
• Prefer: Commitment to continuous learning and professional development.
