Job summary

Legend Senior Living is seeking a dedicated Medication Management Nurse to oversee medication administration and ensure safe, effective use of medications for residents. This role requires strong organizational skills, attention to detail, and a compassionate approach to resident care.

You will pass medications, monitor administration for safety and compliance, educate residents and caregivers, and collaborate with physicians, pharmacists, and other nurses to optimize therapies.

Qualifications

  • Active LPN license in Pennsylvania.
  • Minimum of 2 years of clinical healthcare experience (focus on medication management).
  • Strong pharmacology, medication administration, and drug interactions knowledge.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency with electronic MAR systems.

Responsibilities

  • Pass medications on assigned shift.
  • Monitor medication administration to ensure accuracy, compliance, and resident safety.
  • Conduct routine medication and treatment storage reviews per policy and procedure.
  • Educate residents and caregivers on proper medication use, potential side effects, and adherence to treatment plans.
  • Collaborate with healthcare providers to optimize medication therapies.
  • Maintain accurate and detailed medication records and documentation.
  • Other duties as assigned by Health Care Director.

Requirements
Requirements--Medication Management Nurse
  • Required: Active Licensed Practical Nurse (LPN) license in Pennsylvania.
