Job summary

Mount Sinai Hospital seeks a Pharmacy Intern to assist team pharmacists, patients, and clinicians in effective, safe, and cost-efficient drug therapy. You will handle medication histories, data entry, patient counseling, and care coordination under supervision.

Requirements: completion of the first professional year of pharmacy education, English proficiency (oral and written), and a valid New York State Pharmacy Intern Permit; community pharmacy experience is preferred.

Qualifications

• Minimum first professional year completed at an accredited college of pharmacy.
• Excellent English communication (oral and written).
• Valid New York State Pharmacy Intern Permit required.
• Previous community pharmacy experience preferred.

Responsibilities

• Data entry: obtain medication histories from patients and schedule pharmacist visits.
• Data analysis: payer data and claims.
• Support the pharmacist to implement the patient’s care plan and navigate insurance.
• Provide medication, lifestyle, and disease state counseling.
• Counsel patients and families to adopt lifestyle changes and use MyChart for access.
• Obtain medication lists and discuss adherence barriers with patients.
• Notify pharmacist/provider of missing prescriptions or refill requests.
• Call pharmacies to confirm prescriptions were received and picked up.
• Call patients to remind them to pick up medication.
• Request records from clinics and facilities when necessary.
• Send referrals to other disciplines to coordinate care.
• Enroll patients into criterion-based pharmacy programs and document in EMR.
