Job summary

Albany Medical Center seeks a Medication Historian Technician to collect and document patients' medication histories prior to admission, coordinating with patients, caregivers, and providers to ensure accuracy. You will work under the supervision of a pharmacist in the Pharmacy Department to reduce errors and delays in care.

The role requires 1–3 years of healthcare experience, high school diploma, and certification (PTCB/NHA, NY LPT preferred).

Qualifications

• High School Diploma/G.E.D. required.
• Associate's Degree in Applied Sciences or related field preferred.
• 1-3 years of healthcare experience required.
• PHAR-C - Certified Pharmacy Tech through PTCB or NHA upon hire required.
• PHRMTECH-R - Registered Pharmacy Technician upon hire required.
• LPT - Licensed Pharmacy Tech in NYS upon hire preferred.

Responsibilities

• Collect and document the patient’s medication history from admission to ensure accuracy.
• Record allergy and adverse reaction information.
• Communicate discrepancies to supervising pharmacist and providers.
• Document information in the electronic health record.
• Interviews with patients and caregivers to assess medication history.
