Job summary

Albany Med Health System is seeking a Medication Historian Technician to collect and document accurate patient medication histories prior to admission, coordinating with patients, guardians, and providers. Under pharmacist supervision, you will support the care team by identifying discrepancies, reducing medication errors, and ensuring timely reconciliation across electronic systems.

Qualifications

• High School Diploma or GED required.
• 1-3 years of healthcare experience required.
• Knowledge of pharmaceutical products and potential adverse reactions.
• Ability to read and comprehend medication instructions.
• Excellent written and verbal communication.
• Work cooperatively in a medical center environment.
• Ability to adapt in a fast-paced, complex setting.
• PHAR-C - Certified Pharmacy Tech through PTCB or NHA Upon Hire - required
• PHRMTECH-R - Registered Pharmacy Technician Upon Hire - required
• LPT - Licensed Pharmacy Tech in NYS Upon Hire - preferred
• Equivalent combination of education and experience may be substituted.

Responsibilities

• Collect and document patient medication histories accurately.
• Communicate with patients, guardians, and providers about medications and discrepancies.
• Document all information in the electronic health record.
• Identify and resolve discrepancies with pharmacists and providers.
• Partner with the care team to ensure accurate medication lists and reconciliation.
• Maintain thorough and compliant record-keeping.
