Job summary

Qualifications

• Requires high school diploma or GED.
• Minimum of five years of on-the-job experience in a related role.
• Active pharmacy technician registration with the Georgia Board of Pharmacy and national certification (PTCB); history certificate preferred.

Responsibilities

• Obtains medication histories on all assigned patients during scheduled shifts.
• Ensures prescriptions are sent to Crisp Regional Retail Pharmacy when appropriate.
• Interviews patients to obtain dose and frequency of current medications and supplements.
• Calls community pharmacies to confirm regimens or investigate discrepancies.
• Communicates medication-related issues to health care professionals.
• Writes notes in the electronic medical record documenting the history findings.
• Provides exemplary customer service to staff and patients.
• Maintains patient confidentiality and adheres to hospital standards of conduct.
