Job summary

Trinity Health Mid-Atlantic is seeking a Certified Pharmacy Technician (Medication Historian) for Samaritan Hospital, with evening shifts (11-7 or 1-9). The role involves conducting patient interviews to build complete medication histories for inpatients and ED patients, entering data into the EMR, and coordinating with physicians and pharmacies.

Strong communication and leadership are essential. Minimum 1 year of pharmacy experience and a current CPhT certification are required.

Qualifications

• Minimum education: high school diploma or equivalent.
• Minimum of 1 year pharmacy experience.
• Certified Pharmacy Technician (CPhT) from PTCB or NHA.
• Accurate and detail oriented with strong time management.
• Strong communication and leadership abilities.
• Proficient in English (written and spoken).
• Ability to interact with patients, caregivers and healthcare team.
• Willingness to interact with patients at bedside with PPE.
• Knowledge of top 200 medications and dosing.
• Computers and common software proficiency.
• Standing and walking for long periods.

Responsibilities

• Identify the patient to be seen per hospital policy based on alerts and consults.
• Complete medication history for each medication using a questionnaire to obtain a comprehensive list.
• Document medications, including prescriptions, infusions, OTCs, and supplements into the EMR.
• Document dates and times of last doses administered per protocol.
• Verify medication history with secondary sources (e.g., EMR resources, patient’s pharmacy, MARs, physicians).
• Verify and document patient allergies and reported reactions.
