Job summary

MercyOne Des Moines is seeking a Medication History Pharmacy Technician for the Des Moines, IA area. The role focuses on obtaining and recording accurate medication histories from patients or caregivers upon presentation to Mercy patient care areas, in coordination with pharmacists and the care team.

The position requires completing the competency set, interviewing patients, and documenting medications, allergies, and sources in the electronic medical record.

Qualifications

• High School Diploma or GED; state certification and Iowa registration required.
• National Pharmacy Technician Certification and Iowa Board registration at hire.
• Minimum 2,000 hours of pharmacy technician experience.

Responsibilities

• Obtain, verify, and record initial medication histories from patients or caregivers.
• Conduct patient interviews to document prescriptions, OTCs, vitamins, and supplements.
• Record allergy history and source of information in the EMR.
• Consult with a pharmacist for unresolved uncertainties.

As a Medication History Pharm Tech at MercyOne, will provide technical support in obtaining, verifying for accuracy, and recording initial medication histories from patients &/or their designated family/caregivers upon presentation to Mercy patient care areas. Includes conducting patient &/or family interviews focused on obtaining current medication regimens including prescription and over-the-counter (non-prescription) medications, as well as vitamin and herbal supplements. Perform other functions of a pharmacy technician registered in the State of Iowa and within the scope of Iowa law as needed.

Schedule: 8-hours Evenings 2:00 PM - 10:30 PM

General Requirements:

Successfully complete Medication History Pharmacy Technician Competency/Skill Set. Interview patients &/or their designated family/caregivers as soon as possible after patient presents for care to MercyOne clinical areas (includes the Emergency Department, as well as inpatient and outpatient areas). Obtain & record in the electronic medical record a current list of medications to include: prescription medications, over-the-counter (non-prescription) medications, vitamin and nutritional supplements, as well as the source(s) for this information. Recording should include the following: name/location of patient’s ambulatory pharmacy, medication/supplement name, dosage form, strength, dose, day/time of most recent dose, and source(s) of the information being recorded. Attempt to clarify as many uncertainties and compile a Best Possible Medication History utilizing patient and family/caregiver information, ambulatory pharmacy records, clinic records, long-term-care facility records and information from previous hospital encounters. Obtain & record in the electronic medical record an allergy history for each patient. In doing so, clarify & record the nature of the allergic reaction when necessary. Consult with a pharmacist in all cases of unresolved uncertainties related to a patient’s medication & allergy history.

Education:

High School Diploma or GED National Pharmacy Technician Certification and Registered with the Iowa Board of Pharmacy at the time of hire OR Registered as a Pharmacist-Intern with the State of Iowa at time of hire 2,000 hours of pharmacy technician experience
