Responsibilities

• Performs medication list confirmation for patients admitted to the hospital, compiling a complete home med list including OTCs and supplements with dosage and frequency.
• Coordinates with nursing, pharmacy, and providers to ensure medication lists are updated within 24 hours of admission.
• Interviews to obtain known drug allergies and reactions.
• Documents tasks in the electronic medical record.
• Follows up with community pharmacies and physician offices as required.
• May function as staff pharmacy technician if needed.

General Summary And Purpose

Essential Functions

• Gathers information about the patient's medications from family or directly from the patient in a manner that is professional and non-invasive.
• Works independently to perform medication list confirmation for patients being admitted to the hospital and compiles a complete list of patient's home medications including over the counter drugs and herbal supplements including drug name, dosage, and frequency.
• Coordinates with nursing, pharmacy, and providers to ensure medication lists are updated within 24 hours of admission.
• Interviews to obtain any known drug allergies and reactions.
• Familiar with the outpatient dispensing models and pharmacies patients may be engaged with
• Documents tasks performed within the electronic medical record.
• Follows up with community pharmacies and physician offices as required.
• May be required to function as staff pharmacy technician if the need arises.
