Job summary

Trinity Health in Nampa, Idaho, seeks a Medication Historian Tech to assist with accurate medication lists for patients starting in the Emergency Department and continuing through inpatient units. The role operates under Pharmacy Department supervision and supports care teams across the continuum.

The position requires Idaho Certified Pharmacy Technician licensure and 3 years of experience, with preferred experience or certification in Saint Alphonsus systems.

Qualifications

• Must be licensed as a Certified Pharmacy Technician with the Idaho State Board of Pharmacy.
• Minimum 3 years of experience as a pharmacy technician required.
• Preferred to have 6 months with Saint Alphonsus National Certification (CPhT, ExCPT or equivalent) required.

Responsibilities

• Assist with accuracy and completeness of medication lists for patients across the continuum of care.
• Gather information about the patient's medications from family or patient in a professional, non-invasive manner.
• Coordinate with nursing, pharmacy, and providers to update medication lists within 24 hours of admission.
• Interviews to obtain known drug allergies and reactions; document in the EMR.
