Qualifications

• Bachelor’s degree in applicable field or equivalent combination of education and experience.
• Preferred bilingual English and Spanish.
• Two years of related experience in public health, health outreach, or health education.
• Experience with healthcare organizations, providers, patients, or community-based organizations.
• Community Health Worker Certification or equivalent.

Responsibilities

• Coordinates program activities and travels to program locations to provide on-site support.
• Serves as first point of contact for program information and coordinates responses.
• Conducts personal interviews with residents and assists with pharmaceutical applications.
• Builds and maintains relationships with patients, healthcare providers, and community organizations.
• Confers with Healthy South Texas staff on patient care plans.
• Maintains databases and program materials, including records.
• Composes correspondence, reports, and presentations for the program.
• Reviews and updates program policies and procedures.
• Assists patients in reviewing care plans and overcoming barriers.
