Job summary

ChenMed LLC is seeking a Medication Adherence Technician to engage patients in in-depth conversations about adherence, identify barriers, and coordinate with providers and the pharmacy team to deliver an organized adherence product for improved compliance.

Responsibilities include outbound calls, barrier removal, coordinating refills or 90-day supplies, and ensuring proper processing through the pharmacy workflow. Strong communication and organizational skills are essential.

Qualifications

Responsibilities

  • Remove barriers to ensure prescriptions are provided; analyze, research and resolve issues.
  • Interact with healthcare professionals at all levels about patient medications.
  • Assist with refills, 90-day supplies, and medication synchronization.
  • Coordinate prescriptions through the pharmacy workflow to dispensing.
  • Follow medication adherence workflows and follow up with enrolled or new patients.
  • Meet daily outreach targets for telephonic, email, and mail outreach to patients and clinicians.
  • Support training for clinical personnel involved in PartD HEDIS measures.

Job description

Medication Adherence Technician

This role applies knowledge and skills to engage in in-depth discussions with patients about medication adherence, identify and resolve patient adherence barriers, and coordinate with providers, pharmacists, and the pharmacy department to deliver an organized adherence product that helps patients stay compliant with their medication regimens.

Responsibilities
  • Make outbound calls to patients and healthcare professionals regarding patient medications, adherence, and barrier removal.
  • Work diligently to remove barriers to ensure prescription regimens are provided to patients; this may involve analysis, research, and problem‑solving to obtain resolution.
  • Interact with healthcare professionals of all levels concerning patient medications.
  • Call the pharmacy or provider to assist patient with medication refills, medication synchronization, or 90‑day supply requests.
  • Capture and support medication adherence patient prescription requests/needs.
  • Facilitate and/or coordinate the process whereby prescriptions are sent into the pharmacy for patients, processed appropriately, adjudicated to third‑party insurance, error resolution completed, packaged, and prepared for dispensing to the patient.
  • Follow established medication adherence workflows, including but not limited to following up with patients requesting enrollment and patients currently enrolled to initiate or continue delivery of medication regimens.
  • Ensure assigned prescription processing tasks are accurate and reviewed by supervising personnel as required before dispensing to patients.
  • Be motivated to meet set targets as established by leadership; complete minimum daily goals for outreach (telephonic, email, mail) to patients and clinicians for medication adherence measures.
  • Provide feedback and/or solutions on how to improve the medication adherence patient/client experience.
  • Assist with training programs for clinical personnel involved in PartD HEDIS measures in the centers of ChenMed/JenCare/Dedicated.
  • Perform other duties as assigned and modified at manager’s discretion.

  • Ability to practice discretion and care on a regular basis when working with sensitive/confidential data pertaining to the patient, department, or company.
  • Ability to interact with all levels of internal and external parties, to convey information correctly and always represent ChenMed and its entities in the most favorable manner.
  • Capable of multi‑tasking in a fast‑paced, ever‑changing healthcare environment.
  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database, and presentation software.
  • Ability and willingness to travel locally, regionally, divisionally or domestically up to 10% of the time.
  • Spoken and written fluency in English.
Education and Experience Criteria
  • High School Diploma or equivalent required; additional experience above the minimum may be considered in lieu of the required education on a year‑for‑year basis.
  • Must meet at least one of the following requirements:
    • Certification as a Pharmacy Technician or Medication Assistant with a minimum 6 months of related work experience.
    • A minimum of 2 years of work experience in a medication dispensing/pharmacy setting or call center.
Pay Range

Pay Range: $14.30 - $20.42 per hour. The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
