Insight Global is seeking a Community Based Care Coordinator to coordinate care for individuals eligible for both Medicare and Medicaid in long-term care settings. You will assess medical, behavioral, and social needs, develop individualized care plans, and connect members with housing, transportation, food assistance, and other community resources.

Qualifikationen

• Current, unrestricted clinical license in Ohio (RN, SW, or Counselor).
• Experience in healthcare fields such as Nursing, Social Work, Counseling, Case Management.
• Experience working with dual-eligible Medicare/Medicaid populations.
• Maintain compliance with NCQA and CMS standards.
