Job summary

Insight Global is seeking a Community Based Care Coordinator in Dayton, Ohio to coordinate care for individuals eligible for both Medicare and Medicaid. You will assess medical, behavioral, and social needs, develop individualized care plans, and connect members with community resources.

Work with interdisciplinary teams to coordinate care across providers and community organizations, educate members on benefits, and monitor progress to improve health outcomes.

Qualifications

• Must hold an unrestricted clinical license in Ohio (RN or ASN, MSW, or Clinical Counseling).
• Experience in healthcare fields such as nursing, social work, counseling, case management, or care coordination.

Responsibilities

• Conduct comprehensive member assessments to identify medical, behavioral, functional, and social needs.
• Develop, implement, and update individualized care plans.
• Meet with members in homes, healthcare settings, and community locations.
• Coordinate transitions of care and follow up after hospitalizations to prevent readmissions.
• Serve as liaison between members, providers, community organizations, and service agencies.
• Educate members and families on Medicare/Medicaid benefits and preventive care.
• Maintain accurate documentation of care plans, interventions, and outcomes.
