Qualifications

Position Overview:

The Medicare Coach is a compassionate, proactive, full-time role supporting Medicare members and their caregivers. Working between 8am–8pm EST, you will build trusted relationships, guide health journeys, coordinate care, and support member engagement with healthcare resources. The role involves administering health assessments, coaching on health goals, involving caregivers, and collaborating with healthcare teams to ensure comprehensive, family-centered care.

Key Responsibilities:
  1. Serve as the primary contact for Medicare members and caregivers, providing empathetic support via phone, video, and messaging.
  2. Administer and review health risk assessments to identify priorities.
  3. Coach members using motivational interviewing and SMART goals to promote health and care activation.
  4. Involve caregivers to support whole-family health.
  5. Assist members with healthcare provider engagement and resource utilization.
  6. Collaborate with internal teams and external providers to ensure coordinated care.
  7. Coordinate timely interventions, referrals, and follow-ups.
  8. Document all interactions accurately for care continuity.
  9. Contribute to resource development for Medicare members and caregivers.
  10. Conduct proactive outreach to enroll eligible members.
Qualifications:
  1. 2-3 years of experience in care management, health coaching, or supporting Medicare members.
  2. Preferred: Certification in health coaching (NBC-HWC), RN, MSW, or clinical licensure; note clinical scope is not within this role.
  3. Excellent communication skills, with ability to explain complex health concepts empathetically.
  4. Experience with motivational interviewing, SMART goals, and health planning.
  5. Knowledge of Medicare benefits and healthcare navigation.
  6. Proficiency with digital tools like Google Suite, Slack, Zoom, Apple devices.
  7. Highly organized, detail-oriented, and capable of accurate documentation.
  8. Comfortable with proactive telephonic outreach.
  9. Solution-oriented, independent problem-solver, adaptable to a fast-paced environment.
  10. Multilingual skills are a plus.
Additional Information:

Pay: $33-40/hour, 35-40 hours/week. Benefits include health insurance, paid holidays, parental leave, 401(k), and more.
