MedStar Health in Baltimore seeks a Care Coordination professional to support Medicare FFS beneficiaries attributed to our practices. You will engage in telephonic outreach, close care gaps, and maintain EMR documentation to support the CTO’s care management team.

You will work with Lead Care Managers, coordinate referrals, run monthly reports, and participate in quality improvement initiatives. A CMA license is required and the schedule is Monday–Friday, 8:30am–5pm.

Qualifications

• High School Diploma or GED required.
• 3–4 years of work experience including 1+ years of proven case management and population health coordination experience.
• Experience with data collection and reporting and community outreach experience preferred.
• Familiarity with the local area and population health workforce integration.
• Experience working in an ambulatory setting preferred.

Responsabilités

• Contributes to department goals and adheres to policies, procedures, quality and safety standards.
• Builds and maintains relationships with care teams and community partners.
• Maintains timely documentation in the EMR (MedConnect) and enters data into power forms.
• Manages and coordinates referrals and supports program goals.
• Supports Lead Care Managers and participates in multidisciplinary quality teams.
• Performs other duties as assigned.
