Qualifications

• High school diploma or GED required.
• 3-4 years of case management/population health coordination experience.
• Certified Medical Assistant CMA-AAMA and Maryland CMA license required.

Responsibilities

• Coordinate care for Medicare beneficiaries and close gaps in care.
• Enter data into EMR and run monthly reports for program reporting.
• Support Lead Care Managers and multidisciplinary teams.
