Job summary

LiveWell Homecare Agency, LLC is seeking an RN to deliver direct patient care at home, including assessment, planning, implementation, and evaluation within nursing standards. You will promote patient- and family-centered care, educate on illness management, home care, and medications, and collaborate to develop individualized care plans with the care team.

You will participate in safety, quality, and regulatory activities, and support professional development.

Qualifications

• RN license and nursing degree required or equivalent.
• Bachelor’s degree preferred for advanced roles.
• Two years of experience preferred for advanced roles.
• Commitment to patient safety and high quality care.
