Job summary

Language Fundamentals is seeking a Per Diem Speech Language Pathologist in Loudonville, NY, to join an exceptional professional network focused on high-quality care.

You will collaborate with the rehab team to meet residents' needs, provide restorative therapy, observe FEES as needed, and support patient-centered care under the PDPM framework. CCC-SLP licensure required.

Qualifications

• Graduate of an accredited Speech Language Pathology program.
• Current State licensure as a CCC-SLP.
• Excellent verbal, written and interpersonal communication skills.
• Goal oriented with ability to utilize critical thinking and problem-solving skills.
• Ability to utilize technology to perform electronic documentation.

Responsibilities

• Collaborate with the team of PT/OT, nurses, physicians and dieticians to meet the comprehensive needs of the residents in the facility.
• Provide short team rehabilitation, geared toward a functional return home.
• Provide restorative therapy to improve the quality of life of the long-term residents in the facility.
• Interact with families and nursing staff on patients’ after-care programs.
• Treat under the Patient Driven Payment Model Reimbursement System.
• Observing in the administration of on-site FEES testing for patients with swallowing disorders.
