Schedule: Part‑Time; 10:00 AM‑2:00 PM Monday‑Friday.

Major Responsibilities & Duties

• Facilitates care for the patient through referral to appropriate resources in the community or within the Boys Town system.
• Provides leadership in the identification and acquisition of resources (local, state, and federal) for patients and their families.
• Coordinates activities/follow‑up to meet identified needs of select patients to facilitate necessary care, specialty consultation and/or surgery.
• Serves as a resource for BTNRH physicians, nurses and staff.
• Participates in multi‑disciplinary teams as requested.
• Maintains complete, accurate, and timely documents of completed work, patient files and office files.
• Complies with all applicable statutes, rules, regulations, policies and procedures.
• Maintains working knowledge of relevant medical/legal issues that impact patient care.
• Participates in quality improvement activities through staff meetings, committees, continuing educational and professional activities.
• Participates in Utilization Review Committee and activities.
• Assumes the duties of the discharge planning coordinator. Will provide oversight of the discharge planning process. A daily review of the active patient discharge plans will be completed as needed.
• Reassess the discharge planning process on an on‑going basis (i.e., at least quarterly), review discharge plans in closed medical record review to determine staff responsiveness to the patient's post‑discharge needs and review re‑admissions for trends.

Required Qualifications

• Bachelor's degree in Social Work required.
• Certification, Licensure, or Provisional Licensure as a Social Worker in the State of NE required.
