Qualifications

Responsibilities

• Facilitates care for the patient through referral to community resources.
• Provides leadership in identifying and acquiring resources.
• Coordinates activities/follow‑up to meet identified patient needs.
• Serves as a resource for physicians, nurses, and staff.
• Participates in multi‑disciplinary teams as requested.
• Maintains complete and accurate documents of completed work.
• Complies with all applicable statutes, rules, and regulations.
• Maintains knowledge of medical/legal issues impacting patient care.
• Participates in quality improvement activities.
• Assumes duties of the discharge planning coordinator.
• Reassesses the discharge planning process on an ongoing basis.
