Job description

Shift

Inpatient Medical Social Worker – casual, weekdays 08:00–16:30.

Overview

Seeking a skilled and compassionate Inpatient Medical Social Worker to provide psychosocial support to adult patients and families, assisting with discharge planning, crisis intervention, and care coordination across medical and surgical units.

Responsibilities

• Complete initial bio/psychosocial assessments for adult inpatients to identify needs and develop appropriate interventions.
• Collaborate with healthcare teams to create and implement individualized care and discharge plans.
• Connect patients with community resources such as home care, skilled nursing, and acute rehabilitation.
• Respond to crisis situations including abuse/neglect concerns, substance use, and homelessness.
• Participate in interdisciplinary team meetings, rounds, and performance improvement initiatives.

Required Qualifications

• Master's degree in social work from a Council on Social Work Education (CSWE)-accredited university.
• Minimum of two years of experience in social work.
• Knowledge of professional guidelines regarding confidentiality of client and staff information.
• Proficiency in Microsoft Excel and Word.
• Ability to work effectively with diverse patient groups and families.
• Excellent communication skills, both verbally and in writing.
• High-level commitment, motivation, energy, team orientation, and professionalism.
• Knowledge of health care services and resources.
• Ability to work independently.
• Strong analytical skills.
• Certified Social Worker (CSW) by Nebraska law or Provisional Certified Master of Social Work (PCMSW).
• Geriatric department: Licensed Clinical Social Worker (LCSW) required.

Preferred Qualifications

• Licensed Clinical Social Worker (LCSW) for non-geriatric departments.
