Key Responsibilities

• Assess patients' psychosocial, financial, and environmental needs
• Conduct initial and follow-up social work evaluations
• Communicate significant findings to the care team
• Implement and update the Plan of Care, ensuring quality and safety
• Document and verify the Plan of Care and patient progress
• Provide counseling, education, and support to patients and families

Minimum Requirements

• Master’s or Doctoral Degree in Social Work from an accredited program
• At least 1 year of social work experience in healthcare
• Current State License as a Social Worker
• Excellent verbal and written communication skills
