About the Position:

During visits that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

Matrix Provider – What to Expect:

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as appropriate.
• Coordinate with Primary Care Physician (PCP) on patient education and follow-up.
• Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.

Nurse Practitioner -

PRN/Part Time - Home Risk Assessments

Type: PRN/Part Time - Flexible Nurse Practitioner

Pay: Earn from $2,000 up to $10,000 per month (based on 20-100 completed visits at $100 per assessment). Compensation varies with availability and assessment completion.

Location: Home, SNF, Telehealth, and other community environments in Dublin, Georgia.

Hours: Days, evenings, nights, weekends, flexible scheduling.

Benefits: PRN/Part Time providers are non-benefited except for 401(k) and limited voluntary benefits.

Mileage reimbursement plan included.

Qualifications:

Must Haves:

• Master’s Degree required or equivalent experience with NP licensure.
• Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
• Board certified by AANP, ANCC, or AACCN in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills with patients, clients, and colleagues.
• Flexibility and comfort with frequent change.

Travel Requirements:

• Ability to travel, valid driver’s license, and proof of adequate automobile insurance coverage.
• Travel required to meet patients where they live.
