Job summary

UnitedHealth Group is seeking a Medical Social Worker for Home Care and Hospice opportunities. The role involves providing social work services under physician direction to help patients and families cope with health-related challenges and to support the care team.

Responsibilities include developing a Plan of Care with the Interdisciplinary Team, guiding access to community resources, and serving as Bereavement Coordinator when needed.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Current CPR certification.
• Licensed Social Worker in the state of residence.
• Valid Driver's License and access to a dependable vehicle or public transportation.
• 1+ year of social work experience in a health care setting.

Responsibilities

• Assist the patient, family, physician and health care team to understand social, emotional and environmental factors related to health status.
• Contribute to the development of a comprehensive, integrated Plan of Care with the Interdisciplinary Team.
• Instruct health care team members on community resources available to assist patients.
• Serve as Bereavement Coordinator and establish a Plan of Care addressing bereavement needs.
