UnitedHealth Group is hiring a Medical Social Worker for Hospice services in home-based care. You will provide psychosocial assessment, counseling, and care coordination with the interdisciplinary team to support patients and families in the home setting.

The role emphasizes addressing emotional, social, financial, and environmental factors, coordinating with care teams, and promoting safe care transitions.

Qualifications

• MSW/DSW degree from a CSWE-accredited program
• Active LMSW or LCSW licensure required
• Minimum 1 year of social work experience in healthcare setting
• Meets state and federal requirements; CPR certification

Responsabilités

• Provide psychosocial assessment, counseling, and care coordination in home hospice
• Collaborate with interdisciplinary team to connect patients with community resources
• Support emotional, social, financial, and environmental factors affecting health
• Assist with care planning, transfers, and discharge as part of the plan of care
• Educate patients and families on social work interventions and treatment plan
• Identify risks in patient environment and coordinate appropriate interventions
• Perform other duties as assigned
