Job summary

St. Joseph Hospice in Baton Rouge, LA seeks a Full-Time Licensed Medical Social Worker to join the patient care team. You will provide in-home hospice support, conduct assessments, and collaborate with families and care staff to plan discharge and ongoing services.

Requirements include an MSW, Louisiana license, and reliable transportation. Home health or hospice experience is preferred, with strong communication and empathetic patient care driving success.

Qualifications

• Master's Degree in Social Work required.
• Active Louisiana social work license.
• Experience in Home Health or Hospice preferred.
• Excellent oral and written communication skills.
• Valid driver license with reliable transportation for in-home visits.

Responsibilities

• Complete documentation accurately and in a timely manner.
• Conduct patient assessments and ongoing care for hospice patients.

• Manage casework needs of terminally ill patients and families.

Essential Duties Include

• Complete documentation accurately and in a timely manner
• Conduct patient assessments and ongoing care of hospice patients
• Communicate with patients, families and co-workers
