Job summary

The Carpenter Health Network is seeking a Full-Time Licensed Medical Social Worker to join our patient care team in Baton Rouge, LA. The role focuses on comprehensive hospice care, patient assessments, and coordination with families and staff.

Ideal candidates hold a Master's in Social Work, with preferred hospice/home health experience, strong communication skills, and reliable transportation for in-home visits.

Qualifications

• Master's Degree in Social Work is required.
• Experience in Home Health or Hospice preferred.
• Strong oral and written communication skills.
• Valid driver license with proof of insurance for in-home visits.
• Empathetic, people-oriented professional.

Responsibilities

• Conduct patient assessments and ongoing hospice care for patients.
• Communicate with patients, families and co-workers to coordinate care.
• Manage casework needs of terminally ill patients and families.
• Participate in discharge planning and in-service programs as needed.
