Job summary

Trinity Health At Home is seeking a compassionate Medical Social Worker (PRN) to assess and support homebound patients and families across Washtenaw and Wayne Counties. You will work with physicians and the care team to address social barriers and promote improved health outcomes and independence.

The role requires an MSW from a CSWE-accredited program, state licensure, CPR certification, and at least two years in a healthcare setting.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Current MSW license as required by state regulations.
• CPR certification.
• Minimum of two years of social work experience in a healthcare setting.
• Ability to provide services to acutely and chronically ill patients.
• Strong communication, assessment, and critical thinking skills.

Responsibilities

• Conduct comprehensive psychosocial assessments of patients and caregivers.
• Develop and implement individualized, person-centered plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Identify social, emotional, environmental, and financial barriers to care and connect patients with community resources.

• Collaborate with physicians and healthcare professionals to address factors impacting patient health and well-being.
• Continually evaluate patient needs and modify care plans as appropriate.
• Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings.

Position Purpose

Trinity Health at Home is seeking a compassionate and skilled Medical Social Worker (PRN) to provide consultation, assessment, and support services to homebound patients and their families. In this role, you will help address complex social and emotional challenges that may impact a patient's ability to manage their medical condition, promoting improved health outcomes, independence, and quality of life.
