Job summary

Trinity Health At Home seeks a Medical Social Worker (PRN) to provide consultation, assessment, and support to homebound patients and families, addressing social and emotional needs that impact medical care and outcomes.

You will perform psychosocial assessments, develop person-centered care plans with the care team, connect patients with community resources, and support patients and caregivers through complex emotions to improve quality of life.

Qualifications

• Master’s Degree in Social Work from CSWE-accredited program.
• Current MSW licensure as required by state regulations.
• CPR certification is required.
• At least two years of social work experience in a healthcare setting.
• Ability to serve acutely and chronically ill patients.
• Strong communication, assessment, and critical thinking skills.
• Commitment to patient confidentiality and professional ethics.

Responsibilities

• Conduct comprehensive psychosocial assessments of patients and caregivers.
• Develop and implement individualized, person-centered plans of care with physicians, patients, families, and the care team.
• Identify barriers to care and connect patients with community resources.
• Provide counseling and support to patients and families facing distress.
• Assist patients and caregivers in understanding and following medical recommendations.
• Collaborate with physicians and healthcare professionals to address factors impacting health.
• Continually evaluate patient needs and modify care plans as appropriate.
• Participate in care conferences, quality improvement initiatives, and interdisciplinary meetings.

Position Purpose

Trinity Health at Home is seeking a compassionate and skilled Medical Social Worker (PRN) to provide consultation, assessment, and support services to homebound patients and their families. In this role, you will help address complex social and emotional challenges that may impact a patient’s ability to manage their medical condition, promoting improved health outcomes, independence, and quality of life.

What You Will Do

• Conduct comprehensive psychosocial assessments of patients and caregivers.
• Develop and implement individualized, person-centered plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Identify social, emotional, environmental, and financial barriers to care and connect patients with appropriate community resources.
• Provide counseling and support to patients and families experiencing complex social or emotional distress.
• Assist patients and caregivers in understanding and following medical recommendations.
• Collaborate with physicians and healthcare professionals to address factors impacting patient health and well-being.
• Continually evaluate patient needs and modify care plans as appropriate.
• Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings.

Minimum Qualifications

• Master’s Degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
• Current Masters of Social Work license as required by state regulations.
• Current CPR certification.
• Minimum of two (2) years of social work experience in a healthcare setting.
• Ability to provide services to acutely and chronically ill patients.
• Strong communication, assessment, and critical thinking skills.
• Commitment to patient confidentiality and professional ethics.
