Qualifications

• Master's Degree in Social Work, Psychology, Counseling, or closely related behavioral science field.
• Two years of full-time experience providing counseling and/or social work services to medical patients.
• substitution: Baccalaureate degree with two years or three years of relevant experience as described in the minimum qualifications.

Responsibilities

• Interview families and clients/participants with intensive care management to determine needs.
• Interagency coordination and collaboration.
• Participate in interdisciplinary rounds or team meetings to review client needs and plans.
• Review and maintain chart documentation in a timely, clear manner.
• Provide consultation and recommendations regarding patient care and discharge planning to medical and ancillary teams.
• Communicate with clients with increased psychosocial needs, including crisis intervention and basic needs support (food, housing).
