Job summary

Medi Home Health & Hospice, a division of Medical Services of America, Inc., is seeking an experienced Full-Time Medical Social Worker to support our Hospice patients in Carroll and Blacksburg, VA. You will collaborate with the interdisciplinary team to plan and coordinate patient-centered social work services.

Responsibilities include bereavement support, assessing needs, and serving as a liaison to community resources.

Qualifications

• Master's or Bachelor's level social work degree from a CSWE-accredited program.
• Currently licensed to practice as a social worker in the state of VA.
• Minimum of one-year Hospice experience preferred.
• Valid Driver's License.

• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
