Medi Home Health & Hospice, a division of Medical Services of America, Inc., seeks an experienced Full-Time Medical Social Worker for our Hospice patients in Danville, VA.

The role collaborates with the interdisciplinary team to assess and support patients and families, provides grief assessment and bereavement counseling as appropriate, and serves as a liaison with community resources. Requires MSW/BSW and VA licensure; hospice experience preferred.

Qualifications

• Master's or Bachelor's level social work degree from an accredited school.
• Currently licensed to practice as a social worker in the state of VA.
• Minimum of one-year Hospice experience preferred.

• Provides social work services in accordance with the physician’s plan of treatment.
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
• Serves as a liaison with other community resources.

Job Requirements

• Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
• Currently licensed to practice as a social worker in the state of VA.
• Minimum of one-year Hospice experience preferred.
• Valid Driver's License.
